$45 / Initial with PT evaluation
$20 per 10 sessions on machine

Membership Terms: Monthly memberships are non-refundable. Credit will only be given if prior notice to missed time is given to the front desk (much be at least a consecutive 10-day time period missed). No partial (pro-rated) prices will be given to individuals involved in a couple or family membership. No cash refunds on gift certificates.
